The official entry form for the 2009 World Dairy Expo Championship Dairy Product Contest is below.  A separate entry form must be used for each entry.  An entry fee of $50 per entry is required to enter the contest.

Each submitted entry form must include payment via a check, made payable to Wisconsin Dairy Products Association, or credit card.

All paid entry forms must be received
no later than August 7, 2009
