Shipping tags for contest entries are below.  Both tags must be completely filled out.  The top tag must be placed inside the shipping box and the bottom tag must be securely adhered to the outside of the box.
Each contest entry should be shipped in a separate box.
Contest entries must arrive the week of August 18-21, 2008.
